I would like to see a Contact Us ( or Get Started ) Form / Modal pop-over that aligns with the brand and is visually appealing. i.e. color theme should match company theme and graphic assets should also match theme

The included fields should be:

* Contact First & Last Name - Required
* Email - Required
* Phone - Required
* Project Description (or Message) - Min 50 characters required
* Company Name - Optional
* My minimum budget exceeds 50,000$ - Required - Maybe a checkbox

Buttons:
- Cancel ( or similar )
- Submit ( or similar )

Also to be included:
* Example error message / invalid field
* Responsive design
