I need a DVD cover design for web marketing graphics and print

I will be selling a 6-DVD Piano Instruction Set online. The 6 discs will be together in a standard rectangular size 14mm DVD case (it will hold all 6 discs).

DVD set title: Master The Piano
Tagline: The Ultimate Collection of Step-by-Step Piano Lessons
Color scheme: Use some which go along with my site: PianoKeyz dot c o m

I need:
1) A 3-D image to put on my website showing the DVD case graphic along with the 6 discs next to it or laid out in front of it. This is for marketing so that people will see what they are buying.

2) The .pdf (or other appropriate file type) to print the flat insert which I will put into the DVD case sleeve.

3) A flat graphic of the DVD image so that I can print that onto the DVD itself. For this, please leave a spot on the bottom half of the DVD so that I can put text in myself of the appropriate disc title. For example, even though all 6 discs are apart of the series, each one is a different topic, so I could put in text that says 'Chords Disc 2' for example. Other than the spot to leave for me to add an appropriate disc title, the 6 DVDs should be the same design.

Is DVD case and DVD size is same? Some Cases will be in rectangular shape. Better to specify the size clearly! Thank you.
