One-to-One Service.com provides a customer interaction solution, iService, that contains detailed contact information for customers and prospects. The iService application is used for email response management, live chat, salesforce automation, and other types of customer interaction.

The purpose of the preview dialer form is to streamline the telephone prospecting process for sales staff. The form will identify a list of contacts within the iService database and load them one at a time for the user. The form will present the salesperson basic information about the contact along with the telephone number to be dialed. The first version of the form will required the user to click the number or manually dial it. THIS MOCKUP IS ONLY FOR THE CONTACT PRESENTATION PAGE OF THE APPLICATION.

The preview dialer form will also provide:
- the ability to pick a wrap up code for the call (completed, no answer, wrong number, etc.) and enter short call notes
- display of a short script that the sales person can use during the call
- display of call history extracted from the iService database
- a scheduling function for updating the contact with a target next call date and time.

A hand drawn example of the form that is to be mocked up is attached. Feel free to post any clarifying questions.

Also attached is the product logo, an example of another form, and a Sprite image that contains the various buttons currently used within the application. You can see the product at https://1to1.iServicecrm.com. User of fonts is open, but we prefer sans-serif for web apps.

The form size should be no wider than 1024px.

IMPORTANT INFORMATION
When the mockup is awarded, we will offer the selected designer the opportunity to convert their Photoshop mockup into an HTML page for $150. We hope that people submitting their designs have good HTML / CSS skills to convert their design to HTML. The HTML version will be a prototype, and we will do all the integration work to tie it into the iService Forms interface. After the HTML prototype is complete we will be seeking a third project to convert the single page app into a larger, fully functional AngularJS application.
