Design a landing/squeeze page for my business. It's a weight loss/health niche. It's not just a mockup but the whole project including the psd, html and and all the requirement codes/files to upload on my site. The design must be only one page or above the fold and must be very interesting, unique and able to persuade my message effective.

The structure for my landing page is something like this and can be alter a bit depending on your design:
