Healthcare facility that uses jotform for online intake. Data is manually extracted and entered into an excel log which then prepopulates relevant administrative forms with name, date of birth and date onto 3 or 4 other forms. We have 1 form we would like to add to jotform (hipaa acknowledgement) that is currently signed manually on arrival. We have 2 more forms 1 in word and 1 in excel that we handwrite data on (name, date of birth and date). We would like these forms all in one location (either word or excel) and to prepopulate those fields (name, dob, and date) from the jotform submission so that all we need to do when an intake comes in is hit print. Currently the excel log is designed this way but requires manual data entry from jotform to excel.
