I need to make a dashboard where i can generate some links for surveys to run. Also the status and time should be updated when the survey gets completed. If someone can do so, then let me know.

We created a survey that we sent out to our audeince. The surevey was intended to identify the attributes and demographics information of our core customers. We received roughly 460 response and each responce has roughly 60 peices of information. We want someone to process this data so that we make use of the information. Hello and thank you for taking time to read this. I am looking for someone to create a survey template that monitors health of an individual. Once they answer from a list of 50 or so questions it will generate a report. The reporting will be linked to pre designed answers(paragraphs). So the report isnt always the same there are several choices for each outcome. There must be an ability for these answers to be randomised. The report needs to look clean and well designed. An ability to show the information and where to improve with graphs or some sort of graphic. This project can be done in any format so long as it can be used through a website.
