10 or fewer PDF form fields to fill in with MS Access Data

I have a PDF with home grown fields. I use access to generate a one record table of the information. I need the form upon opening, or button controlled, get the information and fill in the fields with that data. I have done this in the past, but I do not have time to work on it now, and I need a new form made. I can supply the table, and the form to the successful bidder. I need this turned around quickly, within 24 hours preferrably, and since I have accomplished this in the past, I am ceratain someone with PDF javascript experience will find this a simple task.
