466745 Open Cart Checkout modification

We need to change the 'checkout' process form fields.

We need to have 2 sections for the checkout process. I'm including a rough mockup of the layout.

For the top section. The only fields we need to be required are 'Name', 'Email', and 'Phone'. Then we need the 'comments' section under that and title 'Other' We then need the 'continue' button changed to say 'Check Availability' and placed next to this section.

The second section is to be under this and will be optional information customers can add. It should be titled 'Find a Distributor' and should have optional fields of 'Company', 'City', 'State'. It should have a button that says 'Check Availability + Find a Distributor' This functionality will be the exact same submit action as the previous 'check availability' button.

We then would like to know if we can have it go straight to the thank you page and skip the 'confirmation page'.
