I am in need of custom data base and customer forms using File Maker Pro 12.
This will be used for a pet teeth cleaning business.
The customer form in File Maker Pro will in part be modeled from our current customer forms that are used but with a different program.
Attached are two forms the Waiver form and the report card. We would like to merge these into one customer form.
Forms will contain the customers information as well as their pet. Some customers have up to 5 pets.
Forms and databases need to be expandable.
Additional features will need to be added including:
Tracking of appointment and record history.
When customer fills out the form we need the form to indicate that the customer is a returning customer. This does not mean having a form with a check box for the customer to check off if they are a new customer or returning customer, but rather as customer fills out form based on data entered the form can indicate this is in fact a returning customer. We give a discount for all new customers and some customers may say its their first time, but they are not in fact a new customer.
Pricing and notes at the time of the service is provided.
Pricing History: what the customer paid in the past, discounts given etc.
Before and after pictures in each record for each visit. We need to be able to associate each pets before and after pictures with their specific appointment.
