Hi I would like a form designed to simplify our workload. Basically we have written 1 page reports for patients undergoing a particular diagnostic test. Currently we are manually writing these reports for each patient.
I would like a standard report that we can simply change the numerical particulars for each individual via drop down boxes (ie patients DOB, Ht, Wt, etc).
I also want the report to be able to add recommendations based on patients. For instance, for overweight patients, I want to be able to add a recommendation about weight loss.
Ideally these forms are completed in Microsoft Word so that we can save them and individualise them where necessary.
I have attached a sample form for your perusal.
- The yellow highlights are where I want these as drop down boxes with values that we regularly use.
- The red highlights are what I would like to add or not depending on the patient results. I will also need to add more potential recommendations, but these are just a sample.
