Convert Excel Spreadsheet into Employee Metrics

We want to provide our employees with a monthly Performance Metric report card. The report should be visually appealing and a sample is included. The data for the report will be provided in one or more Excel spreadsheets. If there are multiple sheets, we will have an employee ID on each sheet to enable merging.
The end result should be in PDF format with one PDF per employee. We also need a simple way to distribute the PDF's to each employee. Alternatively, we may send batches of the PDF's to each manager. For example, if we have 20 managers, they will receive a PDF for each of the employees they manage.
The sample report card is a first draft and we will most likely update this format as we add new metrics.
The sample Excel sheet included is accurate but will eventually contain new added metrics.
Each employee may also work at multiple locations. They will need one report card per location. In the Excel sheet, each row becomes a report card.
Please describe how this will be accomplished. Is it best to create a website where we upload the excel sheet? Is it best as a standalone software? Or will this be done by Macros or Mail Merging?
