Flash Academic Research Tool

I need an interactive webapp where someone can do the following 1) on a side bar there are "Xs" with a space to enter text--I imagine something like a flash animation game where someone can drag and drop "squares" only you can write in a section on the squares 2) in the main section next to the side bar, there is an X in the far right (or left) center. From it are equi-distant multiple radii cut with equi-distant rings (like a spider web) with labels saying "closest"on the ring closest and "farthest" on the ring farthest away 3) in the main section there should be gridwork radiating from the center X that corresponds to the rings and radi but with smaller units (say 3 or 4 between each ring) 4) people should be able to enter text onto the text boxes under the X's in the side bar and then drag them over to where they want to on the grid in the main section 5) once they've finished they with this section I have a survey of the typical survey format (multiple choice etc) 6) once they've finished with the whole thing, I would like to be able to get an Excel document detailing a) the answers to the questions from the survey section, b) the coordinates of each X with the labels included 7) you need a database back end which identifies each user by an IP address *HASH* (as the user_ID s) 8) This database should store radii datapoint with the information provided (where the small X is in location of the big X for the user_ID) Honestly, I am not sure the best method for storing radii datapoints-- please include this in your response. all entries should be stripped of capital letters and only appear in lower case (This is for compiling purposes so Catholic would be catholic) I recommend two text fields-- the first when they enter the religious belief (Catholic) and then a second in application pop-up that says "Why is <Catholic> where it is?" (Or however you word your questions) and then every 3-4 "Why is Entry.A closer then Entry.C" all of which should be stored and linked to the User_ID Another important factor-- this app will reside in to spots [login to view URL] and then [login to view URL] [login to view URL] will need a password and should user a single unique identified as the user_id Another important note-- this will be hosted on an apache box-- so mysql will be the databse (unless postgre provides something you believe we'll need for this-- we are open but again it is an apache box so MS products won't be considered.)
