Patients of Canada is a social business start-up. I would like branding (logo, website look and feel) developed for a social business. The URL will be [login to view URL] and this URL is curretly parked. The website can be built using WordPress.
Content (text and navigation) for the website will be provided. Initially, only 2-3 pages will be needed. One of the pages will contain an embeded electronic form used by visitors to register for the [login to view URL] service. The number of people who fill in the form will be tallied and the total displayed on the website and the [login to view URL] service will not be initated until a minimum of 500 service subsribers are recruited.
A social business is defined as a business whose purpose is to solve social problems in a financially sustainable way. The term Social business was defined by Nobel Peace Prize laureate Professor Muhammad Yunus; he describes these businesses as created and designed to address a social problem.
Background for Patients of Canada
Patients are in most instances not in control and without easy access to their own healthcare information. The medical errors and negative health outcomes associated with closed information systems are well documented; including, undermining the system’s ability to deliver of patient centered collaborative team based care.
“Health Literacy” is a special term used to recognize a specific form of knowledge. In Canada, “health literacy” is defined as: “The ability to access, comprehend, evaluate and communicate information to promote, maintain and improve health in a variety of settings across the life-course”.
It may be easier to speak of health literacy as the degree to which individuals have the capacity to obtain, process and understand basic health information and services needed to make appropriate health decisions. According to the Public Health Agency of Canada, 60% of adults and 88% of seniors are not health literate.
Currently, there is no service that provides patients with their complete health history and gaining access to a patient’s complete medical history is problematic due to the many geographical and service provider silos that hold a patient’s health record.
