Concept: a website that allows our clients to 1) log into with a password, 2) enter a name and address, and 3) download a PDF document with that name and address entered on it.

for each client, we'd prefill the document with everything EXCEPT the "Certificate Holder" box at the lower left.
we would upload the mostly-filled PDF on the website.
the client would log in with a username and password, they'd type in a name, address, city, state, zip and
that info would populate the "Certificate Holder" box.
they'd then hit "print" or "download" and the filled-in PDF would download for them.
on the backend, the website would log all traffic on the site and would either a) retain the name, address info or b) retain the PDF as printed/downloaded.
the traffic to the site will be absolutely minimal, perhaps 10-20 clients will have access. so the front end can be just a simple log-in screen. we can hand out the log in and password to each of those clients. this will allow us to "automate" the creation of these documents--the client themselves will be able to "create" them.
