We would like someone to design a form that our customers can use to send us a break down of their income and expenses.
Here is the contents of the form.
*Each area is itemised and requires a numeric entry.
*Once completed the data will feed into our CRM (our CRM provider will map these fields).
*It would be helpful if next to each field there was a further field which the customer can add the frequency of income (Monthly, 4 Weekly, 2 Weekly, Weekly)
*We are looking for a user friendly and simple to use form design.
*We would need a Previous, Next and Submit buttons to navigate around through the pages.
It would be great if we can see any examples of previous/similar work are available.
