I'm looking for a way to collect Dates Closed information for members of my consortium. The end goal is to have a list of dates for a particular organization which will be used to update a table located in a Microsoft SQL Server (updates to the server are NOT included in the webpage).
1) User goes to webpage where a year is displayed in calendar format.
2) User enters their identifying information (library name, email address).
3) User clicks on dates to indicate they plan to be closed.
4) On submission, the information is sent to a system admin, with a copy being sent to the user's email.
Required: Long term viability (little or no maintenance needed to update the year), multiple dates permitted
Desired: 1) User can click a header to select all the weekdays in a month/year (all sundays, for example). 2) They can subsequently de-select individual days if necessary. 3) Holidays are indicated on the calendar (US holidays first, other countries optional). 4) User selects identifer from a drop down (which we can update in the code ourselves at a later date, if needed). The results email will then include additional information based on the identified submitter.
