162818 Search form design

I need a new search form designed for a travel site. The form should have a "calendar style menu" so that it is possible to pick from three different search options (flights, hotels & packages). Each search option has a different set of form fields that will be provided. I need an experienced web designer. Please post a link to your portfolio. Please make a note in your bid if you can also modify the form to display current dates and the possibility to pick dates from a calendar. I can have someone else do this later though if I otherwise like your design work. PMB if you need to know the site the form will be integrated in. Thanks, Patrick
