It is a good option to have it built from scratch using JS , rather than opting for Survey JS or other libraries, since doing it from scratch can incorporate all the features specific to this Survey. I checked your pdf file and understood about your motive of providing this questionairre for visitors. But I still need to get something clear. Thats why, we need to contact. My Approch: I will use Pure Javascript as this will put great value to the website performance. In these kind of works, we dont actually need libraries. But If I really need to use any third-party libraries, I can use them too. (only If you want) Some issues/questions: 1. The questionairre form is a little bit of an old design. If you dont mind I will design it and improve the UI with CSS and other stuff. 2. Can you please tell me more about the scoring feature? Because I still dont get how the scoring will be. 3.
