Magento USPS Shipping Method, add option / force Insurance

We run a Magento store in which everything is shipped via USPS. We ship valuable products which need to be insured.
Currently Magento does not offer any options on the built-in shipping method for USPS to use insurance.
We would like an extension that would 'enhance' the built-in method so that it will calculate the price depending on what zip code is entered (like the original functionality) including the insurance amount calculated from the cart price. The insurance costs can be calculated from a table, but the USPS API does offer the pricing.
We would prefer an installable extension to ease installation.
