Write 2 workflows for a simple list, and create one form that will drop into sharepoint list when submitted.

In SharePoint 365, I need the following assistance.

I would like to create 2 simple workflows for the same list. Once a item is changed from no to yes, it will send a alert to the item creator.

I would also like to create a simple form that will drop into the same list once created.

I will need to know your hourly rate, as well as a estimate on how long it will take.
