Web Page to Email Delivery System

Looking for a web programmer/developer: I have a service webpage, where users of the site can submit a job description via a form (with various fields such as...job description, photo upload, services needed, name, email...etc.). upon submission, the form data needs to be captured inside a mysql db for referencing and an html email needs to be sent to all service providers of the requested service, and a separate email to be sent to the site user for confirmation. Email requirements - The emails must display all of the form data (including uploaded photos) either displayed within the email or in link form whichever in easiest. - The emails must be css equipped ( i will handle the css design of it) - Must be able to add banner advertisements within the email.
