Need to setup automatic form posting script (PHP based)(repost)

I need a script setup to take information out of a mysql table and submit it to the form located at [login to view URL] This form has an geotargeting restriction so only US based connections can access it. This means if you are outside of the US you will need to connect via US proxy to see the form. The server the script will run on is US based so there should be no complications there. Your script must be able to communicate to the above URL, grab the hidden form variables located in the form itself, then post both those variables and the email address field (populated with an email address taken from the mysql database) and the checkbox field agreeing to their terms and conditions marked as checked. The script must verify the submission completed and once completed the script will then update a field in our mysql table indicating the email was submitted. We have consent to submit information to this site this way, we just need the technical work done to make it possible. The script must be written in PHP, use Curl and be properly commented for future developers to understand the functionality. The script will be tested for accuracy and functionality before payment will be released. ## Deliverables 1) All deliverables will be considered "work made for hire" under U.S. Copyright law. Employer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the employer on the site per the worker's Worker Legal Agreement). 2) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 3) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Employer's environment--Deliverables must be installed by the Worker in ready-to-run condition in the Employer's environment. b) For all others including desktop software or software the employer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this project. ## Platform Web Php/MySQL
