121914 Create pages for PHPList

We have installed PHP List on our server but we want to have the subscribe, confirmation, unsubscribe pages fit our site's design (2 websites) and move one or two buttons on the page.

All that is needed is to create the subscribe pages (header and footer) and modify the public pages (header and footer).

Prefer someone with good design skills and PHPList experience.
