I need a script that will send an email including multiple fields including an upload form. I must be able to read the code and plug it into an existing html page, and add new html form elements. The sending is to an email address that can be configured in the php file. Make a reasonable offer, this has been done before but I want something thats easy to plug into an existing website look and feeel. Ideally you would have code in a separate php include file and use some basic tags to call functions in the include, just so my html file isn't too messy.
