I have a simple online form to sign up for a public seminar, located here:

When they fill out the form, I get an email with the information.
We currently have almost 40 sales reps working on this marketing campaign.
I'd like to enhance this online form in three ways:
(1) That it would write the data to a text file
(2) need an admin screen that allows me to view the list, and then to download the list
(3) Each time an entry is made, 3 different emails would go out to the sales teams. Here's what I would like it to do:
(Step 1) I'll have a text file in the folder that has 4 columns:
sales rep name, sales rep email, supervisor email, my email
(Step 2) When the customer types in the name of the sales rep, the script will look at the data table and find the name on the list of sales reps. It will then send an email to the sales rep email, the supervisor email, and to my email.
If the customer does not type in the sales rep name, or, if they type something that is not on the list, then the email would just go to me.
I'd like this to produce a form similar to the one on the page. But, I would want to be able to modify the form as other events pop up.
It would be great if we used like a php header and footer to separarate out all of the html code currently on the form page.
