Hello, We need to build a form same as: [login to view URL] in the style of our site [login to view URL] with the header and footer from our site on our site. The form should email all forms submitted to gea at elikaassociates.com. Once the user selects send then a similar to that of [login to view URL] should be enabled that we can edit with the appropriate message. Also the user should receive an automatic email that we will need to provide content for that will confirm receipt of the form etc. Looking at completing this asap.
