We are doing a survey and need to have an Excel Sheet created to track the results.
The Excel Sheet will mirror the survey which will be provided in .doc format.
We need formulas in the data receptacle cells to continually tally responses and give the appropriate cumulative data. With ratings we need average rating as well as total number and % of the whole for each response. For yes/no, and check the box we need the total and % for each choice. For verbatim comments we need space to have the verbiage input. We will ultimately be grouping these by comment categories - determined by the nature of the responses.

1) Complete and fully-functional working Excel 2003 Workbook tailored to our survey tracking needs.
2) Deliverables must be in ready-to-run condition with Excel 2003 for PC, without purchasing or installing additional components or software.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
