Student survey project

I am a student conducting a survey study. for this study I have prepared HTML pages. three in all - the introduction, the survey and the final thankyou page. I want to post these pages online so that people can take part in the survey online. alternately u may wish to prepare the survey in the same format on your own. Once the survey is take i want the user completed information mailed to me. also the third page has optional information which if filled in must be e-mailed to me. there are three buttons in all - one on each page. the button on the introduction page loads the survey page and likewise the submit button on the survey page loads the thankyou page while mailing me the info. and finally the submit button on the thakyou page simple mails me the info filled in. The site finally loaded must be named as csrsurvey2004 preferably on geocities but other free website may also do with a minimum of pop up windows. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform As this is a public survey it must be Accessible via anymachine on all operating systems.
