Needed email form, with email received in readable layout

A mailing form that formats my information into readable email message.

Php with or without MySQL is prefered.

Mybe someone has a such a formtemplate lying around, which can be modified to my needs.

The reason for a layout is the space reduction with printing, so the archive doenst get too big.
