We need someone to create a form which we will be able to use for our party events which will allow clients to enter these fields:
and one blank field to enter "additional guest"
Once Submitted, the results should go into an excel sheet in an organized manner by being in alphabetical order by First Name.
We need this to created once with full guidelines so we can reuse it for every event. This is a sample of what we do right now which we are not happy about at all. We would like something in a form that we can use over and over.
In case we have multiple events we should be able to have a drop-down menu so the client can choose the event he would like to be a guest for.
I know this is a quick fast process so we are looking for someone who is experienced and can get the job done in one run. Thanks!
