Autocomplete + Foursquare API

URGENT

Need to do a FORM to create a record on a mySQL database.

The form will have the following fields:

location:

location:

venue:

category:

picture:

(users need be able to upload a picture)

note:

The venue field has to be loaded with autocomplete, suggesting names of venues from Foursquare API (see the example at [url removed, login to view] and example_form file attached).

If the venue is not found the user will be able to create a new venue. Please see how they do on foodspotting, the link to create a new venue is the first result shown at the autocomplete. Also check the fields to create a venue in the picture attached (create_venue)

Need to be coded in Javascript, php and mySQL. All the communication with the database has to be asynchronously (AJAX).

I'm building a bi website and might have more projects in the future, so this can be the beginning of a good partnership.
