449247 FAST - simple javascript/ajax/jquery work

Hello, I'm outsourcing a piece of code I'm working on because I'm tired of debugging it :). I am building a login form. I took this sample code from [login to view URL] and am turning it into a login form. The ultimate goal of the login form is to have forgot password and registration functionality open in the same modal box using AJAX and jquery (good example is [login to view URL] login). The error checking for missing or incorrect data should remain as is in the example. Looking for a strong, precise and experienced Javascript developer with AJAX and PHP expertise.
