200954 Response form link from e-mail

I need a form with a few questions that is linked from an html e-mail message. I need to receive the response to this form by e-mail.

I will send e-mail, target will click link in e-mail message which will open the html form; The target will complete the form and press send and I will recive the response by e-mail.

The form will automatically be cleared for the next person .

The e-mail and form, will be in english and french.
