103886 Simple 1 or 2 page PHP site

Hello folks,

This one's a quick job. I need a very simple one or two-page php website with a simple calculator function built into it. Users should be able to store a number corresponding to their email address.

If a user comes to the site for the first time, he will enter a number and his email. When he clicks "Submit", a record (Email and Number) is stored in a table, and an email is sent to the user with a link.

When the user clicks on the link, the number stored earlier should be retrieved and displayed to the User. The user will then make a couple of other entries for calculation

purposes, and the numbers returned from the calculator are displayed to the user.

The look of the site is ready (HTML + CSS), you only have to put in the PHP/MySQL stuff, and the javascript(?) required for the calculator function.
