Survey - creation, taking, and result analysis

I need to get a MySQL db driven and PHP/Javascript coded survey creation to be taken on web and ipad or iphone via web, results displayed in some graphic manner..
See tables in excel file and some basic forms..but I need someone to create forms etc .. like Web 2.0 look and feel..
A simple admin interface where only admin can login to add users..
http://weburl/admin/ for admin
https://weburl/ goes to landing page for survey to login
a Thank you page when survey is done all questions.. if someone stops in middle then next time they login it should start from that place.
