First time poster here, so bear with me!
I'm looking for someone to install a pre-made upload script & email form onto a basic web page. I've chosen to use an upload script called Uploadify and would like to add a simple email form so I get notified when an upload is successful. I have very limited knowledge of HTML5, PHP & jQuery (learning it as I go) and decided to pass this along to a pro who can just get it done properly...I've tinkered with it enough! I would like to use their new 'UploadiFive' (HTML5 Version) of the script and I have purchased a license to use it. I will email the winning bidder the purchased script along with their open source Flash version in zip format.
The HTML5 script says it has a 'fallback' option so when folks try to upload using an older unsupported browser (e.g., Explorer, Safari), it will default back to the original Flash based uploader. This is important to us & I would like this to happen as we have many clients that still use older browsers. I'm not sure what's involved in this but it doesn't seem too complicated from what I've read in the docs.
I would like the uploader installed on a basic, blank webpage with no graphics or flashy stuff - just basic (I can add color & text later). The uploader will have a basic form which the form data will be validated first->upload completes->form data/file info is emailed to me. Uploaded files will land in a named & dated folder. The form fields will include:

• Client Phone:
• Client Email:
• Alt. Email: (**Optional) [No Validation Required]
• Session Notes: (**Optional) [No Validation Required]
Upon each successful upload, form results will be emailed to my email address (I'll give that to you), 'Client Email' & 'Alt Email' (if entered). Email results will look like this below - HTML format preferred but text OK (Example data included):

• Remote IP: [url removed, login to view]
• Browser Info: Mozilla/5.0 (Macintosh; Intel Mac OS X 10_6_8) AppleWebKit/534.52.7 (KHTML, like Gecko) Version/5.1.2 Safari/534.52.7 (Or similar data so I know what browser they used at least)
• Artist Name: Pearl Jam
• Client Name: Ed Vedder
• Client Phone: 555-555-5555

• Alt. Email: brendan(at)[url removed, login to view]
• Session Notes: These are the notes...

As noted above, each group of uploaded files will land in a named & dated folder using the form data:
Artist Name-Client Name-Client Phone-Client Email-Date/Time

I should note our site & server runs on Hostgator Linux CentOS with all of the updated software. We're using an SSL certificate for this upload page, so it's important all uploads are done securely - I don't think that changes anything in the scripts though. The Uploadify scripts allow for different options (e.g., multi uploads vs. single uploads, file size/type limits, etc.) which I'll have to go over clearly with you with what we need. If some of this stuff can't be done or formatted as noted above, I'll understand - I'm trying to give the best image of what I need so I can get accurate bids from everyone. We're a small recording studio, so we don't have a huge budget, but this will most definitely lead to other work down the road. Also, although I'd like to get this implemented soon, there's really no rush for completion as well. Thanks for your interest!
