157380 Ftp Upload Form

We need a simple form for our clients that would enable upload of larger files by ftp protocol to our webhost and email info about client upload to our email.

Form with info will have around 5 fields. It should prevent uploading certain file data - allow upload of only defined file ends. It should run on our linux server (PHP / mysql). If possible a second version should be also delivered as joomla component.
