Phpfox is a social networking site similar to myspace. It is written in php with a mysql database.
At the moment people can post a comment to another person's profile which appears instantly on their profile. This is done by a mini html editor (see attached screen shots).
We'd like to create an additional feature called ï¿½Eulogies' whereby visitors can complete and post a small survey about that person on their profile. The results of which could go in a new section on the left of the profile.
So the form they can complete which will be posted to the user's profile should have the following questions. we might want to alter these in the future.
The thing I like most about them isï¿½.
Their personality traits areï¿½
They could try harder onï¿½.
Their worst habit isï¿½.
The funniest thing they've ever done isï¿½.
Their best asset isï¿½.
My best moment with them wasï¿½.
The person or thing they are most like isï¿½

Their look, in 20 words or less isï¿½.
All that needs to be done from a programming perspective, I guess, is for you to replicate the existing comments section but give it set fields to be filled out rather than an editor for them to use. So the existing comments feature is staying and this will be a new feature on the left of the profile.
I've attached a screen shot of the site as it is so you can see the existing layout, and also another screen shot with a VERY ROUGH mock up of what I'd like this to look like (this is done in Paint so really is just rough). The comments section at the moment just goes onto more pages as more comments get added ï¿½ and so, the eulogies section should do the same.
I'd like the layout of each posting to be as per the attached file, roughly.
