I need a custom web based database application that runs on MySql. The application will have 2 sides, client and admin.
On the client side the user will answer a series of questions by filling out a form.
On the admin side: The admin will get an email that someone filled out the form. They can then login to see the results and filter results by category.
For example: We have four categories
I want to be able to filter by these main categories.
There will also be dynamic categories. When the fill out the form they will list thier city. So in the admin section I'll need to filter by
- Chefs -- City
- Hosts -- City
The admin section should also have a "notes" section so the admin can write comments about person. Also an "approved" function. So the admin can log in and see all the categories but filtered by wether or not they have been "approved" or if they are "pending".
The application should have multi user login. That way we can have:
"New York User"- They would log in and see all of the Contacts for New York.
"Los Angeles User" - They would log in and see all of the contacts for New York.
The master admin could log in and see all contacts in all areas but should be able to filter by location and contact type.
