PHP contact form script with database check (simple)

Hi There! I need a simple PHP solution. First please create a (very simple) form with these fields: 1. Country dropdown (please add a few example values: USA,Canada,Hungary) 2. Postal code textbox 3. City textbox 4. Address textbox 5. VAT number textbox (optional field) SUBMIT button Number 5 is optional, all other fields must be filled out by user before he can click "Submit". User will get a link from me (that will be given to user by me, you don't have to worry about this). Example link user will get: [login to view URL](at)[login to view URL] As you can see a variable (email) is passed to the php. PHP script should do this: 1. Check if Database1(mysql) already contains this email. If email is present, user should be redirected to page: [login to view URL] 2. If email is NOT present, php script should check if email exists in Database2(myql). If email is NOT present in Db2, user should be redirected to page: [login to view URL] 3. However If email is present in Db2, the form from above should come up. And user can SUBMIT the data. Data should be uploaded to Db1. So basically it's a webform that each of our members only has to fill out once. But first it will check whether they really are members (by the email variable).If they are not members (email not found in db), they will be redirected to "not found" page. If they are members and have already filled out the form, they will be redirected to our members' area. If they haven't filled it out yet, they will fill it out. It is a very simple script, you just need to make sure you follow all instructions. I can read php, so I will check the script myself. It needs to be secure.
