Quick form script needed

I need a script written in that will send the results to me using CDO as sendmail or windmail is not available. It s windows server. I tried a perl script with no success. So I need someone who can create the form to send me the results of the form. It needs to be tested and I'll need to test it successfully before payment is made. Payment via Paypal. Project budget is $20.
