Polling System

I need a polling system. Preferably built with PHP. I have included a visual presentation outlining the requirements for this project, please download the attached file. I need this project in as little time as possible, please indicate the estimated amount of time you will need when bidding. After Project Bonus will be awarded based on good work and timeliness. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Installation package that will install the software (in ready-to-run condition) on the platform(s) specified in this bid request. 3) Exclusive and complete copyrights to all work purchased. (No GPL, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site). 4) Any revisions or omissions to or for this project must be agreed to by myself. I reserve the right to not accept any work, if it is not to my satisfaction, incomplete, or not designed/built as I have required. If you do not agree to this, please do not bid. ## Platform Prefer PHP, other language must be approved first.
