PSD to HTML conversion - $110 budget

I have a PSD template that I need converted to HTML, together with Javascript and PHP functionality. It is VERY important that the HTML you give me is easy for me to modify in the future, so that means that as much design as possible has to be in the CSS. I will also require you to upload the code to my hosting server, and set up a "Contact Us" form. Detailed instructions will be provided via video.
Milestones:
Bare-bones template conversion (header+footer) - $20
CSS formatting of all various text formats throughout the website - $20
Homepage conversion (includes automatically-playing video and long sales copy) - $20
FAQ, About Us, and Contact pages - $10 each ($30 total)
Set up Contact Us form to e-mail the form to me - $10
Upload all HTML to the server and make sure it works as intended - $10
