Website Homepage with Banner Placements No Joomla/Drupal/Wordpress

Hello!
I am looking for a website developer and designer to help build a homepage for my new site. This project is for the homepage only. If the work is sufficient then we will work together to build the rest of the site. I am working on starting a health site called Condition Clinic. I would like the feel of the site to be consistent with the current industry:

Here is a list of everything I would like to include in the homepage:
-Clean and clear framework with fixed width
-Leaderboard banner space holder at the top of the page
-Navigation bar with the following buttons: Home, Conditions, Lifestyle, Articles, Videos
-Search bar to the right of the navigation bar and below the leaderboard banner
-Slider including four slides (picture on the left, header on the right top, brief description below title and a click through link)
-Medium Rectangle banner space holder to the right of the slider
-Welcome note including a header and then 4 line paragraph below the slider
-Health Channel scrolling frame two conditions wide
-Lifestyle Center including three small pictures with headers: Diet & Nutrition, Senior & Aging
-Wide Skyscraper banner space holder to the right of the welcome note and lifestyle center, below the slider.
-Leaderboard banner space holder at the bottom of the page below the wideskyscraper banner and lifestyle center
-Bottom bar containing the following links: Home, About Us, Privacy, Terms of Use, Advertise, Site Map
-Copyright note

I have included the corporate logo as an attachment. I am pretty happy with the colors in the logo but could go a shade lighter or darker based on your recommendations. Please note this is a collaborative effort and your feedback is requested. You are the professional and know what is best.
