Build/Develop/Design A Website that offers multiple services, by multiple professionals, in all USA Zip Codes

I need a website built that has 5 service categories visitors can chose From, then subcategories that are all like landing pages, meant to capture a lead/form. Each subcategory will be assigned to a different person in different cities/zipcodes. For example, if a category was Massage, and subcategories were "Deep Tissue Massage Therapy" , "Walk on Massage Therapy" and "Relax Massage" - The visitor would click on an ad, land on the Massage Category Page, enter their zip code, and click on Deep Tissue, they would be led to a Deep Tissue massage landing type page that when a form/lead gets generated, it would go to the therapist that I assigned for that zip code. This page will be for professional services. I want to be able to assign leads and make changes to assignees whenever I want, easily, in all of the USA. Attached is a better idea of what it is. A professional that pays for rights to leads generated in the zip codes I assign them, should be able to log in to a back portal and see all of the leads they have received, down load them in csv or xls format, and click ok which ones actually purchased their service. Each page will have a video that will introduce each professional in each landing page for each Market A general video will be used for markets not sold. But I should be able to upload a personalized video on each page whenever I want, also remove professionals, Add professional, elect what zip codes they get, add categories and sub categories in the future etc. Example sites but not exactly and only limited to my 5 categories for now are [login to view URL] [login to view URL] [login to view URL]
