Create a Custom PDF from Machform

We created this form [login to view URL] using Machforms. When the users completes it the system saves it and gives the option to download it as a pdf (printed form). We need to manipulate this output , so that the pdf looks like ([login to view URL]), so basically add formatting and break the information in sections. Here are some highlights: 1. This form [login to view URL] which is the one the people will be filling up, is fine the way it is and it has already being approved by the client. So we are not supposed to make any changes here…if by any reason you might need to make modifications please let us know so we can discuss. 2. Every time a user fills up the form a unique entry is generated, and so will be the PDF they create; these entries are accesible thought the control panel of machforms. If possible we would like to add the # of entry on the PDF, so that we can reference it back to the corresponding entry in the machform panel. 3. Machform offers the option to generate an email to the client and the user every time a form is completed, we would like it to stay this way. If the option is to have our own system them we would like to have notifications available so that the client gets and email with a link to download the pdf safely. 4. Fields like “Do you have any children” are optional and they became accesible only if the user selects “Yes”. In this case we would like to have this section appear at the end of the PDF. 5. All the documents that get attached on the first question “Generating Data List” should be added at the end of the PDF. 6. On the last questions where there are multiple choice options and radio buttons, it is necessary that the questions with all the possible answers not only the chosen one shows on the PDF like it shows on [login to view URL] 7. Each title with the blue label and white letters is the beginning of a new section. The sections are (Generating Data List, Financial Profile, Expenses, Relative Comfort Scale, Investor Profile Questionnaire y Risk Tolerance Questionnaire) 6 total 8.
