Create a prestashop module to "apply for credit"

My customer wishes a Prestashop module which allows applying for a credit. 2 web pages must be update: The first is located to the details item web page where the item is display with prices and details. (image 1) The second is located to the last checkout web page where you must choose a payment method. (image 2) For futher information tou can see the process in the following links : [login to view URL] or [login to view URL] Details: The amount to apply for a credit must be more than 750 MUR for one item or for the total of all item in the cart. The minimum amount by month is 75 MUR. You must display the month available for the credit ( 6 , 12, 18, 24, 30 month). You must add NIC ( National Identity Card) in the image 2 to apply for the credit. For all news customers you must pay 20% from the total amount including rates. For existing customers the 20% must be optional. The variables which must be update in the back office : The rate. The initial rate is 12% from the price of each items.
