Help with PHP script error

Need help with PHP script on one of my clients sites. The form allows the site visitor to log in and view up to 2 pictures of themselves. The error is coming into play when the Admin enters the form information. It is coming up with the follwoing error message:
Duplicate entry '4294967295' for key 1---insert into customers (first,last,phone,cost,adate) values ('Jimmyer','jankenson','8889990101','143.00','2004-4-19')
Also need one input box added to the Admin form for "comments"
You must be able to communicate with me via MSN Messenger during process.
