Rewrite a small scrip from Perl to PHP

Job Description:
I'm using a PHP plugin but only one file (the start file) is in Perl and it's located in the cgi-bin folder.
Sometimes my server has problems running Perl cgi scripts properly,
so in order to be able to use the plugin safely anytime, I think it would be a good idea to translate it to PHP.
So the job is to translate one small file from Perl to PHP, keeping exactly the same functionality.
A big part of the code (line 22-76) is html so the actual Perl script part is quite small.
The respective file is attached.
