Search and Return a value from a table, to be used on a webpage

We want to be able to import a simple table that can be searched by users.
Users would enter an ID number (4 digit) + the year of their date of birth, it would return results from 1-3 columns in the table.
For example, a person with ID #9889 and dob 9/28/1946 enters 9889 in one field and 1946 in the other, it then returns
